Localisation
- 01Located 0.5 cun superior to the suprasternal notch, centered in the depression there.
- 02At the midpoint of the supraclavicular fossa.
- 03On the midline, centered in the suprasternal fossa, 0.5 cun above the suprasternal notch.
Puncture
- 01Transverse technique: begin with a perpendicular insertion of 0.2 to 0.4 cun, then redirect the needle downward behind the manubrium and in front of the trachea, advancing roughly 1 to 1.5 cun. This produces local distention and soreness along with a tightening feeling in the pharynx.
- 02Perpendicular insertion of 0.3 to 0.5 cun, producing local distention and soreness.
- 03Caution: With the transverse approach, take care not to insert too deeply, as this risks puncturing the aorta or the innominate (anonyma) arteries. Too deep a perpendicular insertion will puncture the trachea. Keep the needle from drifting far to either side — in emphysema patients especially, deviation can puncture the Lung or the subclavian artery.
- 04Puncture perpendicularly to 0.2 cun first, then angle the needle tip down along the back of the sternum for a further 0.5 to 1 cun.
- 05Moxibustion may be used.
- 06Perpendicular insertion to 0.3 cun.
- 07With the patient's neck extended, insert perpendicularly 0.2 to 0.3 cun at first, then guide the needle downward along the posterior edge of the manubrium for 0.5 to 1 cun.
