Localisation
- 01On the lower rim of the eye socket, about one quarter of the way from its outer edge toward the inner edge.
- 02Along the inferior orbital margin, at the point where the lateral quarter meets the medial three-quarters.
Puncture
- 01Perpendicular insertion. Have the patient look upward while you stabilize the eyeball from below with a finger, then slowly advance the needle slightly medially and upward, toward the optic nerve, to a depth of 1 to 1.5 cun. This produces distention and soreness throughout the eye along with a sensation of protrusion. Caution: take care not to insert too deeply, to avoid entering the cranial cavity.
- 02Have the patient close the eyes and look upward. Direct the needle first slightly downward, then perpendicularly, passing between the eyeball and the lower orbital wall, to a depth of 0.5 to 1 cun. Caution: 1. Advance the needle slowly, without lifting, thrusting, or rotating it. 2. Immediately after withdrawing the needle, press firmly with a cotton ball for about a minute to prevent hematoma. 3. Only practitioners with suitable clinical experience should attempt this technique.
